Need Help setting up Server Status plugin or script.

Hello, Is there anyone out there that is familiar with this plugin or script? I have an example pic. If there is someone who can do this pls message me or tell me what it'll take to get it done. I'm also looking for someone fluent in TeamSpeak to the server status and some permission... Need Help setting up Server Status plugin or script.
